To get started, please complete the initial application form using the link provided above. This form collects basic information and will be reviewed before next steps are offered. Families will either be added to the waitlist or offered a spot.

Please note: all families must be registered on London OneList in order to be considered for a space or added to the waitlist.

If a spot is offered and accepted, you will be invited to complete a more detailed registration form and the following steps before your child can begin:

•  Review and agree to the Parent Handbook requirements
• Upload your child’s immunization record to Lillio and email a copy to shots@mlhu.ca
• Submit participation bond cheques (dropped off at Lambeth United Church)
